Overall Responsibilities:

PCG is seeking a highly motivated and dependable individual to conduct home and community based services, non-residential assessments for the Wisconsin Bureau of Adult Long-Term Care. The assessments will be conducted in the non-residential setting utilizing the format put forth by the Centers for Medicare and Medicaid (CMS) Final Rule. The Community Site Surveyor (CSS) is a full-time position and must have experience conducting interviews with individuals that are intellectually or developmentally disabled.

Hours are flexible, including regular working hours and early mornings, evenings and weekends. Ideal candidates will have a social services background with disability populations in community environments, and/or relevant social work, therapist/counseling, or nursing advanced degree or licensure (MSW, LCSW, LMFT, LMHC, RN, etc.).

This person may be located in the Madison area, the Eau Claire area, or the Green Bay area.

Specific Responsibilities:

  •  Clearly communicate purpose and scope of the federal rule requirement to providers and individuals with disabilities.
  •  Conduct site visits to provider agency service delivery settings to assess staff interactions with individuals in the non-residential setting for compliance of the new federal standards for home and community-based services settings.
  •  Consistency in completing survey instruments and to take accurate notes based on observations and regularly report results and information collected back to the project team.
  •  Represent PCG/SSO and the client in a professional manner.

Required Experience:

  •  Associateís degree required, Bachelorís degree preferred. There is potential for greater compensation for candidates with advanced degrees and/or license.
  •  Strong social services background, preferably with home and community-based services supporting populations such as individuals with physical and intellectual disabilities.
  •  Prior experience conducting interviews with individuals with intellectual disabilities.
  •  Survey, assessment, or compliance experience in human services or a related field.
  •  Prior experience conducting health care facility site visits is preferred.

Other Requirements:

  • Site visit scheduling is mostly flexible, with 1-3 hour site visits potentially from 7:00am - 5:00pm, CST, 5 days a week. Some scheduling constraints may apply to get broad sampling of visits at different times and to ensure that visits can be completed within required time frame, at the discretion of PCG.
  • Community Site Surveyors are expected to use their own vehicles for travel, with mileage reimbursement. In-state regional travel will be required to conduct site visits but will be assigned and scheduled in an attempt to cluster site visits to nearby applicants. Overnight travel is not anticipated, but there is potential for an overnight stay if needed.
These roles will be one-year contract opportunities from January 2019 through December 2019.
