Registered Nurse Case Manager (Bilingual)

Overall Responsibilities: 

PCG is currently seeking qualified Case Managers to be located in our Albany, NY office or to work remotely to help launch an exciting new healthcare administrator project. Case Managers will have responsibility helping program recipients, family members, and caretakers navigate healthcare benefits offered through a publicly administered Medical Indemnity Fund on behalf of a state agency. The purpose of the Fund is to provide a funding source for future health care costs associated with birth-related neurological injuries. Members of the Case Management team will act as the Fund Administrator to handle day-to-day Fund operations, including, case management services and prior approval of certain healthcare services and communicating approvals to our claims administrator team.

Members of our team will help develop program policies and procedures and be given the opportunity to make a lasting impact on program recipients by identifying and making improvements on how the program is managed and care management is delivered today. Successful candidates will be accountable for the full patient care cycle. Case manager duties include assessing, planning, implementing, monitoring and evaluating actions required to meet the client’s health and human services needed. A certified case manager is a plus, but not a requirement.

 Responsibilities:

  • Coordinate and provide care that is safe, timely, effective, efficient, equitable, and client-centered
  • Handle case assignments, draft service plans, review case progress and determine case closure
  • Travel within the State of New York to conduct periodic in person assessments
  • Facilitate multiple care aspects (case coordination, information sharing, etc.)
  • Help patients make informed decisions by acting as their advocate regarding their clinical status and treatment options
  • Develop effective working relations and cooperate with medical team throughout the entire case management process
  • Interact with patients to keep track of their progress and to ensure satisfaction
  • Record case information, complete accurately all necessary forms and produce statistical reports
  • Promote quality and cost-effective interventions and outcomes
  • Assess and address motivational and psychosocial issues
  • Adhere to professional standards as outlined by protocols, rules and regulations

 Required Skills:

  • Excellent communication, interpersonal and client-facing skills.
  • Ability to work independently and on a team.
  • Bilingual: Spanish & English 

 Required Experience:

  • Proven working experience in case management, including as a nurse, medical, mental health care manager or a related job
  • Experience working with pediatrics is a plus
  • Experience or educational training in health or social services, preferably including work experience or a practicum that involved the performance of assessments and the development of case management plans.
  • Excellent knowledge of case management principles, healthcare management and reimbursement
  • Previous experience with psychological aspects of care
  • Effective communication skills
  • Excellent organizational and time management skills
  • Familiarity with professional and technical emerging knowledge
  • Problem solving skills and ability to multi-task
  • Compassionate with teamwork skills
  • Current New York State license, certification or registration as a Registered Nurse 
  • BS degree in related healthcare field preferred
